Today, I received the Supplementary Payments Application forms and while I haven’t filled it out and sent it off yet I did have a quick look. Good news for English speakers is that some English instructions have been included. Not everything though as you can see in the images I have provided in the download link below. These images are scans of my application form and can’t be used by other people to apply for this supplementary income. The redacted areas just cover my personal details.

If you are not sure about how to fill in the application form leave a comment here or in the forum and we will try to help you out.
Sending Money Home from Japan
There are several ways to send money home from Japan but most of them are costly. One of the best deals that I have found is golloyds. There is 2000 yen charge (not including bank charges) to send money home and the service is available to most countries in various currencies. Check official site for details on how to apply for this service.
Official Site: (English/Japanese)
Printable Application Form: (English/Japanese)
There is a “Member-get-member Progamme” so dont forget to ask your friends if they are using this service and if they are be sure to put their name on the application form next to “Introduced by…” so they can receive 2000 yen from golloyds.
